LA «COMUNITÀ DI TIPO FAMILIARE» COME SETTING GRUPPALE NEL TRATTAMENTO DELL’ADOLESCENTE BORDERLINE

RIASSUNTO

Il lavoro presenta l’esperienza di conduzione di un gruppo di coppie di genitori di figli adolescenti con disagi diversificati (condotte da isolamento; sofferenza mentale media; rifiuto dell’aiuto o drop-out). Il gruppo, che è stato avviato nell’ambito di un Centro di Terapia Familiare dell’ASL Città di Milano, nasce dalla necessità di individuare uno strumento clinico che possa permettere un’esperienza trasformativa comune a coppie di genitori incontrati per colloqui di consulenza o terapia, in carico ai conduttori del gruppo, a colleghi del Centro o a colleghi di altri Servizi. L’indicazione terapeutica al gruppo è stata individuata nella necessità di offrire alle coppie la possibilità di sperimentare una condivisione delle loro problematiche genitoriali con una modalità, quella di gruppo appunto, analoga a quella che i figli adolescenti prediligono in questa fase evolutiva. È emerso un marcato isolamento relazionale e l’assenza di una significativa rete sociale e spesso familiare di riferimento. Il setting gruppale offre maggiori stimolazioni alle coppie, potendo contare sulle interazioni orizzontali, oltre che su quelle verticali con i terapeuti. L’ipotesi infatti è che, in analogia con quanto avviene con i figli adolescenti, le osservazioni rivolte da altri componenti del gruppo appaiono ai familiari più accettabili e meno colpevolizzanti. Il gruppo omogeneo rispetto alle problematiche familiari ci è sembrato adatto alla condivisione delle difficoltà relative alle trasformazioni dell’identità familiare che l’adolescente e, parallelamente, i genitori sperimentano in questa fase evolutiva. Il gruppo accoglie unicamente genitori in coppia, è co-condotto da una coppia eterosessuale di terapeuti e non intreccia relazioni coi figli. Nell’articolo vengono approfonditi aspetti metodologici attraverso la descrizione dell’evoluzione della dinamica gruppale del primo anno di trattamento.

SUMMARY

A therapeutic group of adolescent children’s parents. The experience of leading a group of parent couples with adolescent children showing diverse problems isolation conducts, average mental suffering, help refusal or drop out is presented. The group has been set up in a Family therapy Centre at Milan ASL, in the effort to prompt a variable clinical instrument likely to enhance a transforming experience for parent couples looking for consultation or therapy and entrusted to the group therapists or to other therapists in or outside the centre. The group was expected to offer the parent couples the opportunity to share their problems as parents with the other members of the group itself, as adolescents often do. A strong sense of isolation and the absence of a meaningful social and family support was shown by the couples. The group setting is highly stimulating for the couples because of the interactions evenly taking place among the members and of those with the therapists themselves. As for their adolescent children, comments from members of the group are usually better accepted by the couples and less likely to make them feel guilty. A homogeneous group of people sharing the same sort of family problems has been considered as better suiting the need for a change in the family identity the adolescents and their parents likewise feel at this stage of their growth. The group is only meant for parents in couples and is led by a heterosexual couple of therapists. It is thoroughly unrelated to the couples’ children. Methodological aspects are dealt with in the article through the de- scription of the evolution of the group dynamic relationship during the first year of treatment.
